We are looking for a Triage Support Coordinator to work on an ongoing contract for a Housing Association based in Weston-Super-Mare.

The organisation are a large provider of social and supported housing across Bristol and Somerset with a head office in Weston-Super-Mare.

The role is to take referrals from the Bristol Housing Support Register, complete an assessment of each individual and then refer them to the support service appropriate for their needs.

Applicants will have a track record working within supported housing, be familiar with conducting assessments and referrals and ideally have a good understanding of the Bristol Housing Support Register.
