Transformation Manager

Key Responsibilities:

To assist as appropriate with the development and implementation of the CCG's strategic transformation plans

To strategically develop and programme manage the delivery and implementation of a number of Integrated Care initiatives and to ensure successful outcomes as integral components of the CSP

To select and lead a project team, liaising as necessary with the CCG portfolio leads and CCG Clinical Leads

Manage the budget, monitor expenditure and costs against deliverables and realised benefits as programmes develop

To provide monitoring and evaluation reports, advise on progress to senior management, transformation subcommittee and the CCG Board

Ensure the delivery of new products and/or services from the projects is to the appropriate level of quality, on time and within budget, according to the programme plan

Take responsibility for the quality assurance and overall integrity of the programme

Develop and write new pathways, processes, protocols and policies to support change management work arising within projects and programmes

Develop clear and specific change management plans to support project delivery and work with staff to ensure that changes are implemented successfully

Contribute to the development of a CCG 'pipeline' of transformation and innovation projects which will deliver over the next 2-3 years
