Social Worker - Adults Continuing Health Care

Our client has a job vacancy for a Social Worker to work within an Adults Continuing Health Care Team based in Doncaster. This vacancy has an initial 6 month contract and the pay rate for this role is £27 per hour (limited company worker).

Requirements:
Have you got a social work degree or equivalent?
Are you HCPC registered?
Do you have at least 2+ years recent post qualified experience working within Adult's Services in the UK?
Do you have a good understanding of the role of social care professional in respect of Continuing Health Care?
Do you have a good understanding of the reviewing process and the Continuing Health Care criteria?
Are you able to negotiate with Health Care colleagues?
Do you have a sound knowledge of mental capacity or best interests?
Do you have experience of dealing with service users with Learning Disabilities?
Are you able to use CareFirst?
Do you have good IT skills?

Duties:
- You will be undertaking Continuing Health Care reviews.
- You will be supporting and advising service users.
- You will be managing a caseload of individuals.
- You will be carrying out needs assessments.
