Our client is looking for an experienced Senior Mental Health Social Worker, required to work in a Community Mental Health setting providing a co-ordinated approach to the assessment of service users and to develop and review individual care plans.

Role and Responsibilities:
1. To undertake the assessment of service users and to develop and review individual care plans within the Care Programme Approach in a multi-disciplinary setting.
2. Demonstrating an understanding of the Recovery Model and Social Inclusion.
3. To undertake care co-coordinator responsibilities for an agreed number of service users.
4.0 Undertake Mental Health Act assessments in the role of Approved Mental Health Professional if approved is desirable but not essential.

Skills & Abilities:
1. Candidates should have worked in a community Mental Health setting and held a senior Social Work position for at least 2 years.
2. BA (Hons) Social Work, DipSW, CQSW or equivalent
3. HCPC Registration
