Registered Nurse - Case Manager Job

REGISTERED NURSE - CASE MANAGER needed for a contract opportunity with Yoh's client located in South San Francisco, CA.

THE BIG PICTURE - TOP SKILLS YOU SHOULD POSSESS:

- Case Management Experience
- Knowledge of Medicare and Medi-Cal

WHAT YOU'LL BE DOING:

- Perform comprehensive assessments and care planning
- Communicate with members and/or family/responsible party to initiate and develop individualized care plans
- Contact identified high risk members and member families to establish plan of care with understanding of goals as developed with PCP; provide clinical and liaison services, including follow-up of discharge from ER and acute/skilled inpatient admissions
- Assist members with appointments for specialists, educational classes, and transportation
- Perform other duties

WHAT YOU NEED TO BRING TO THE TABLE:

- Current California RN license
- Certification as CCM (Certified Case Manager) or equivalent is preferred
- Bachelor's degree in nursing or a related health or social services field is a plus; PHN preferred
- Strong working knowledge of case management, Medicare, and Medi-Cal benefits, as well as the complexities of working with the elderly and disabled populations
- Experience utilizing personal computer and with Microsoft Office products (Word, Excel, Access and Outlook)
