Registered Nurse Case Manager Job

Yoh has a contract opportunity for a REGISTERED NURSE CASE MANAGER to join our client in Foster City, CA.

JOB OVERVIEW:

Our client is currently seeking a qualified candidate to work as a Care Coordination RN Case Manager in a health plan office setting. This is a Full time contract position.

JOB RESPONSIBILITIES:

- Perform comprehensive assessments, develop individualized care planning, initiate and coordinate case conferences with providers.
- Will identify high risk patients and develop care plans.
- Make referrals to various departments for necessary involvement (ie Care Coordination, Disease Management, Health Education, Quality Management and Provider Services when medical condition or needs are appropriate.).
- Review prior authorization requests for medical necessity utilizing established Medicare and Medi-Cal guidelines.
- Assist members with appointments for specialists, educational classes, and transportation.

Job Qualifications:

- Previous experience in managed care setting with emphasis on case management or utilization management required.

Preferred Job Skills/Experience/Knowledge:

- Bachelor's Degree in Nursing or healthcare-related field is preferred.
- Current California RN license required; CCM is preferred.
- Strong working knowledge of Medicare and Medi-Cal guidelines, and previous experience with initiating TARS is preferred.
- Previous experience working with elderly and disabled population is preferred.
