Physical Therapist-Home Health

Position Summary:

The Home Health Physical Therapy Case Manager is a professional caregiver who plans, provides and coordinates patient care in collaboration with the patient, caregiver, physician, and other health care team members. The case manager assesses patients for appropriate admission to home care or other services and facilitates access for treatment as appropriate. The case manager must be able to demonstrate the knowledge and skills necessary to provide services appropriate to age groups according to specific chronological age, developmental age, and low psycho-social maturity.

Minimum Qualifications:

Education: Graduate of an accredited Physical Therapy program with a BS or certification in Physical Therapy. Knowledge of Medicare regulations required.

Required Length and Type of Experience: Two (2) years outpatient physical therapy and one (1) year of homecare experience required.

Required Licensure, Certification or Registry: Current Ohio licensure and CPR certified. Must drive and have a valid Ohio driver's license.
