Nurse Practitioner Job

NURSE PRACTITIONER needed for a CONTRACT opportunity with Yoh's client located in Lexington, Nebraska.

THE BIG PICTURE:
- Home Visit 30 hours a week

Top Skills Should You Possess:
- 1-3 years of Experience in Home Visits
- Medicare Experience

What You'll Be Doing:
- 30 hours a week home visits for Medicare Patients
- Ability to obtain reliable transportation in order to complete home visit assessments
- Knowledgeable in regards to standard of care of common chronic health conditions as well as health promotion and prevention guidelines

What You Need to Bring to the Table:
- Nurse Practitioner in State of Nebraska (Active and in Good Standing)
- Two to five (2-5) years of Experience Home care or home visit experience
- Nurse Practitioner Licensure in State of Nebraska
- Family, Geriatrics, or Adult certification required
- Must have Strong communication skills with the geriatric or Medicare population
- Must have reliable Transportation and be willing to go to different home visits
- Must have a clean driving record from DMV; Must have proof of Car Insurance
- Must have a Clean Drivers Licensure in State of Nebraska
- Prescriptive Authority in State of Nebraska
- Professional image and behavior are required
- Ability to navigate on the Internet and work with personal computer
- Knowledgeable in regards to standard of care of common chronic health conditions as well as health promotion and prevention guidelines
