NHS Continuing Care Manager

IQ Public Sector is looking for an NHS Continuing Care Manager for a role with our NHS client in London.

The ideal candidate will have extensive experience within the CHC arena and will be used to being part of case management panels as well as liaising with stakeholders to develop the local CHC strategy.

You will have worked on individual case management as well as having experience with "fast-tracking" cases.

The post holder will be responsible for co-ordinating the whole process of care assessment on behalf of our client until the decisions have been made regarding individual funding cases has been reached.

You will work with the Children's services to ensure that the transition from children's to adult care is of a high standard and in line with national frameworks.

You will be responsible for ensuring that work is compliant with national standards as well as securing appropriate packages for individuals who are eligible for CHC funding. This will involve working with budgets as well as engaging with clinicians and senior managers to facilitate the appropriate care is provided.

The post holder will also be responsible for providing expert advice for the procurement of CHC services as well as securing the provision of services, if required.
