Medical Social Worker in an Acute Care Setting Job

MEDICAL SOCIAL WORKER (MSW) needed for a CONTRACT opportunity with Yoh's client located in FOUNTAIN VALLEY, CA.

TOP SKILLS SHOULD YOU POSSESS:

- Must have a Masters Degree in Social Work
- Two-Four (2- 4) years of Social Work Experience in an Acute Care Setting
- Must have at least Two (2) years of Case Management and Discharge Planning Experience

WHAT YOU'LL BE DOING:

- The social worker provides discharge planning, information and referral services, counseling, and the coordination of services which meet the health related needs of patients of all ages
- The major functions include assessing, planning, coordinating, implementing, monitoring, educating, and evaluating care
- Ability to mediate between conflicting desires and points of view to bring patient/family/health care team to a mutually beneficial resolution
- The social worker interacts with physicians, residents, medical students, nursing staff, staff from other departments/services involved in patient care, as well as with providers and community agency representatives

WHAT YOU NEED TO BRING TO THE TABLE:

- Master's degree in Social Work from a four-year college
- CPR/BLS Card
- Two-Four (2- 4) years of Social Work Experience in an Acute Care Setting
- Must have at least Two (2) years of Case Management and Discharge Planning Experience
- Must have InterQual Experience
- Intermediate level use of MS Word, Excel and Outlook
- Strong communication and interpersonal skills
- Must be able to work Mon- Fri 8-430 including all holidays
