This is an exciting and unique opportunity to work for a dynamic service which continues to make significant investment in social work. The service is committed to safeguarding and promoting the welfare of vulnerable adults and expects all staff and volunteers to share this commitment.
The Role
You will be required to attend MDT meetings with OT's / Physios and assist with providing assessment and care management to range of individuals being discharged from hospital. This is an acute hospital ideally the person should have previous experience of working within ACS.
This will require you to conduct follow up visits for a 2 week period after discharge to check on patient that could be with family or residential homes. A desirable candidate will have wide background knowledge especially in mental health and physical disabilities
You will be expected to safeguard vulnerable adults, all work is to be carried out within the framework of the seven social care outcomes: Improved health and emotional wellbeing, improved quality of life, making a positive contribution, exercising choice and control, maintaining personal dignity, economic wellbeing and freedom and freedom from discrimination.
Candidate minimum requirements:
Enhanced DBS
HCPC registered
Car driver/ owner
Post Qualified experience working within this setting
