FULL TIME HEALTH RISK MANAGER RN OR LVN needed for a contract opportunity with Yoh's client located in Long Beach, CA.

What You'll Be Doing:
- Complete telephonic health risk assessment with Medicare members. Would not be managing members long term, just the assessment and moves on.
- Under supervision, works collaboratively with Molina Members and the interdisciplinary healthcare team to assess, plan, coordinate and provide education and individualized, person-centered care to achieve optimal clinical, financial and quality of life outcomes. Collaborates with the members.
- Utilize a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote members decisions related to achieving and maintaining optimal health status.
- Conduct standardized Health Risk Appraisals (HRA) and applicable assessments, including the verification of medical history and current health and wellness needs.
- Participate in team case conferences to ensure collaboration to optimize interventions for clients highest risk members
- Standard business hours 8AM-PM

What You Need to Bring to the Table:
- Zero to two (0 - 2) years of experience in health care setting or customer service experience or Social Work, Psychology, Gerontology or related social services field.
- Must be an LVN or RN with health care setting and excellent customer service
- Familiarity with NCQA standards, regulations and measurement techniques.
- Ability to establish and maintain positive and effective work relationships with coworkers, clients, members, providers and customers
- Preferred Managed care setting experience.
