Continuing Healthcare Assessor - Northampton

Our client has a job vacancy for a Continuing Healthcare Assessor to work in Northampton. This vacancy has a 3 month contract and the pay rate for this role is £24+ per hour (limited company worker).

Duties:
- Supporting the Continuing Healthcare Specialist Nurse or Clinical Lead.
- Ensuring that the work stream is planned and managed effectively.
- Making sure Continuing Healthcare guidance is followed.
- Coordinating and facilitating multidisciplinary assessments and patients reviews.
- Undertaking timely reviews for patients who have care packages or placements in place.

Requirement:
- All applicants must be a Registered Learning Disabilities Nurse with a valid NMC pin.
- Applicants must be educated to degree level in a relevant subject, or have an equivalent level of experience of working at a similar level in a specialist area, ideally Continuing Healthcare.
- You must have at least 2 years recent post qualifying experience working as a Continuing Healthcare Assessor.
- You must have experience of working effectively with clinicians, social care and other partner agencies.
- You must also have a good knowledge of CHC Packages and of the CHC Framework.
