Community Nurse (B5) (6mths FTC)

Location: Derby Rd Surgery, Ipswich

Hours: 18.75 hours per week

Suffolk Community Healthcare provide a service to people who live in the community and who require nursing and therapeutic healthcare such as palliative/end of life, IV antibiotic therapy, wound management, phlebotomy, catheter management or rehabilitation support with life-long conditions.

We are looking for a Community Nurse (Band 5) within Suffolk Community Healthcare. Working in the community and in-patient settings you will be responsible for the following:

* Manage a defined caseload of patients / clients within the Local Health Care Team locality, in-patient setting, day and treatment, community
* Assess patients` needs and make appropriate plans for care, implement care plans and evaluating effectiveness
* Work as part of a multi-disciplinary team
* Participate in internal rotation including weekends and bank holidays
* Previous relevant nursing experience, ideally within the community
* Good knowledge of current NHS/Adult Community Services developments
* Ability to work autonomously
* Flexible working to include weekends and bank holidays
* Ability to travel to rural locations
* Full UK driving license
* Level 1 Registered Nurse
* Current NMC registration
