Care Management Coordinator Job

CARE MANAGEMENT COORDINATOR needed for a contract opportunity with Yoh's client located in San Francisco, CA.

THE BIG PICTURE - TOP SKILLS YOU SHOULD POSSESS:

- Knowledge of Medical Terminology and Coding
- Excellent communication skills

WHAT YOU'LL BE DOING:

- Responsible for obtaining and accurately entering data for authorizations, assessments and other data elements to track programmatic and member activity
- Conduct initial telephone interview for patients referred for post-discharge follow up
- Respond to information and assistance requests from patients, their families, physicians and internal customers
- Communicate appropriately and timely all issues that require escalation to supervisor of other member of Care Management team
- Communicate regularly with contracted Health Plans, Behavioral Health organizations, medical director, and care management team and any other provider when appropriate

WHAT YOU NEED TO BRING TO THE TABLE:

- High school diploma; some college preferred
- Familiarity with medical terminology, coding and managed care concepts
- Excellent verbal and written communication skills and analytical abilities
- Computer experience with Word, Access, Excel, PowerPoint and Visio
- Ability to use independent judgment
