Care Coordination RN Job

CARE COORDINATION RN needed for a CONTRACT opportunity with Yoh's client located in South San Francisco, CA.

THE BIG PICTURE - TOP SKILLS YOU SHOULD POSSESS:

- Inpatient and Discharge Planning experience
- Knowledge of Medicareprovide clinical and liaison services, including follow-up of discharge from ER and acute/skilled inpatient admissions
- Assist members with appointments for specialists, educational classes, and transportation

WHAT YOU NEED TO BRING TO THE TABLE:

- Current California RN license
- Certification as CCM (Certified Case Manager) or equivalent is preferred
- Bachelor's degree in nursing or a related health or social services field is a plus; PHN preferred
- Acute/Inpatient background/experience
- Inpatient (initial and concurrent review) and discharge planning experience
- Knowledge of Medicare, and Medi-Cal benefits, as well as the complexities of working with the elderly and disabled populations
- Telephonic and strong customer service skills
