Care Coordination Registered Nurse Case Manager Job

CARE COORDINATION RN CASE MANAGER needed for a CONTRACT opportunity with Yoh's client located in San Mateo, CA.

TOP SKILLS SHOULD YOU POSSESS:

- Knowledge of Case management, Medicare, and Medi-Cal Benefits.
- Geriatric Experience.

WHAT YOU'LL BE DOING:

- Perform comprehensive assessments and care planning.
- Communicate with member and/or family/responsible party to initiate and develop individualized care plans.
- Contact identified high risk members and member families to establish plan of care with understanding of goals as developed with PCP; provide clinical and liaison services, including follow-up of discharge from ER and acute/skilled inpatient admissions.
- Review prior authorization requests for medical necessity utilizing established Medicare and Medi-Cal guidelines.

WHAT YOU NEED TO BRING TO THE TABLE:

- Valid California license as a Registered Nurse.
- Bachelor's degree in nursing or a related health or social services field is a plus.
- Two (2) years clinical nursing experience or equivalent.
- Geriatric experience and familiarity with community resources helpful.
- Experience utilizing personal computer and with Microsoft Office products (Word, Excel, Access and Outlook).
- Candidates must have own vehicle and valid CA driver's license with proof of insurance in conformity with state law minimums as these positions may require travelling.

BONUS POINTS! OTHERWISE KNOWN AS PREFERRED QUALIFICATIONS:

- Bilingual skills (Spanish, Tagalog or Chinese) highly preferred.
- Certification as CCM (Certified Case Manager) or equivalent is preferred.
