Band 6 Care Co-ordinator - Hampshire

Our client has a job vacancy for a Band 6 Care Co-ordinator to work in Hampshire. This vacancy has a 3 month contract and the pay rate for this role is 24-26 per hour (limited company worker).

Duties:
- Care co-ordinating care needs.
- Planning and implementing evidence based care.
- Working with a defined group of service users and their carers.
- Providing professional advice and support to other agencies.
- Working within a multidisciplinary team.
- Establishing therapeutic relationships with service users.
- Ensuring education and support is delivered to a high standard.

Requirement:
- All applicants must be a Registered Mental Health Nurse, with a valid up to date NMC pin.
- Applicants must have at least 2 years recent post qualified experience working as a Band 6 Care Co-ordinator.
