Band 5 Community Psychiatric Nurse - Older Adults

Our client in Wiltshire has a job vacancy for a band 5 community psychiatric nurse to work within a older adults community mental health team. This vacancy has an initial 3 month contract.

Duties:

* Liaising with multidisciplinary teams, service users, families, carers, statutory and voluntary organisations, occupational therapists, day centres and residential and nursing homes
* Maintaining a high standard of individualised client care, recognising factors and events that are likely to affect the mental, emotional and physical well being of clients/carers
* Developing and maintaining a high standard of practice based on assessment
* Attending weekly older adults community mental health team clinical meetings to provide the team with relevant information in relation to caseloads
* Managing a client caseload within a defined clinical area of the community mental health team
* Contributing to all health and social care assessments for those clients on the outreach day service caseload

Requirements:
All applicants must be a registered mental health nurse with a valid NMC pin. You must have at least 2 years recent post registration experience working within mental health.
