1 year Fixed Term Contract

Based: Ipswich, travel throughout East Suffolk

Salary: 21,478 - 27,901 benefits

Suffolk Community Healthcare is an innovative and transforming provider of community health services within Suffolk. Our vision is for a flagship service to support the shift of higher acuity care from acute hospitals into the community setting. This includes clinical interventions that will prevent hospital admissions and that will facilitate patients being cared for earlier in their recovery following hospital admission in the community setting.

As an employee focussed employer, over the past 24 months, we have supported our existing healthcare professionals with the introduction of initiatives to enhance their working day, including our Care Coordination Centre and Community Intervention Service.

We currently have an exciting opportunity for an experienced Band 5 nurse to work as part of our Admission Prevention Team of Community Intervention Service for a 1 year fixed term contract to cover maternity leave.

Key Responsibilities:

* Identify, assess and implement interventions to prevent unnecessary admissions or support discharges
* Use established pathways and processes to provide a wide range of nursing and social care assessments to patients who may be at risk of hospital admission.
* Ensure appropriate assessment, planning and implementation of care and assist in the interpretation of information relevant to the patient's condition/progress, identifying needs/ patient issues and setting realistic goals.
* Responsibility for pro actively monitoring patients' health and social care needs and delivering clinical interventions, so that the individual receives the best possible care in the most appropriate place.
* Provide advice to other professions outside and within the multidisciplinary teams across organisational boundaries.

Relevant Experience:
- Significant post registration nursing experience
- Experience of multi disciplinary/multi agency assessment and care planning
- Experience of providing health education

Relevant Qualifications and Skills:
- RGN Qualification and current NMC registration.
- Clinical skills to include IV cannulation, administration of IV antibiotics, Venepuncture and male

catheterisation
- Excellent level of skills in both verbal and written communication
- Car driver
- The Admission Prevention Service operates a 24/7 service, you must be able to work shifts to accommodate this service.
