Telephonic Registered Nurse - Case Manager Job

TELEPHONIC REGISTERED NURSE CASE MANAGER needed for a CONTRACT opportunity with Yoh's client located in Mission Hills, CA.

THE BIG PICTURE - TOP SKILLS YOU SHOULD POSSESS:

- Nurse Case Management experience
- Ambulatory Care experience

WHAT YOU'LL BE DOING:

- Will review and evaluate ambulatory referrals on all patients for medical necessity using established clinical guidelines
- Work closely with the utilization management directors to ensure that decisions are consistent with guidelines
- Will prepare cases for weekly UM committee and clinical case reviews
- Will work closely with the skilled nursing facility team to deliver appropriate care at the nursing home level
- Assist the UM compliance manager in preparing for health plan audits
- Ensure that all referrals are evaluated in a timely manner

WHAT YOU NEED TO BRING TO THE TABLE:

- Degree from an accredited nursing school required
- Experience in Case Management required
- Clinical experience in an ambulatory care setting
- Current RN license in California
- Current CPR/BLS
