Support Worker

Our clients based in North London require support workers to work within their rehab service. The service aims to keep people independent and in their own homes, by providing rehabilitation and other support services. You will work alongside community rehabilitation teams (i.e. Occupational Therapists & Physiotherapists) District Nursing & Palliative Care.
The service the team provide is guided by care plans, written by health care professionals (OT's, PT's & SW's).

Referrals are made from the following teams:
RAPID RESPONSE - Urgent response for up to 10 days providing support within the community in order to prevent a hospital admission.
SUPPORTED DISCHARGE - Supportive rehabilitation care for up to six weeks, following a recent admission to hospital. Involves an emphasis on enabling care.
PALLIATIVE CARE - End-of-life care and support provided to clients and their families within the community in the last four weeks of life.
PACE - Post Acute Care Enablement - A five-day care package for clients discharged early, with full support at home from health professionals.

Duration: Ongoing
Hours: shift work (7 day service, 7am - 3pm or 3pm - 10pm, some night shifts are available for those that want to work nights 10pm - 7am)
