Role Responsibilties
You will work effectively with team colleagues to provide and co-ordinate support to ensure compliance with:
Regulatory standards as defined by legislation and regulatory bodies
Contractual standards for each purchaser
Best practice as identified within the organisations policies and procedures 
You will also support people and co-ordinate their support as outlined in their person centred plan. At all times this should be done in such a way to ensure it places the person at the centre of decision making. You will also provide and co-ordinate the following:
Personal Care
Domestic Household Support
Social Support
Family Support
Leisure Support
Relationship Support
Medication Support
Liaison with Health Professionals and Social Care Professionals
Personal Safety
Landlord liaison and support
Liaison with appropriate agencies concerning maintenance of property and services
These are just some of the areas where you may be required to provide direct support or co-ordinate support with other team members. Details of the actual areas will be covered within an individual&rsquos personal plan, their person centred review and notes of a keyworker session.  At all times support should be in line with this plan, the regulatory guidelines for the country you work within, the purchaser&rsquos contract and the organisations policy and procedures. If unsure ask your line manager or another organisational manager.
