Projektbeschreibung
A professional caregiver who plans, provides, and directs patient care and teaching, utilizing the nursing process in accordance with patient care standards and physician orders. He/she acts as a liaison between physicians, patient and family, and collaborates with other health care members. He/She participates in health center and department committees. The staff nurse must be able to demonstrate the knowledge and skills necessary to provide care appropriate to age groups according to specific chronological age, developmental age, and/or psycho-social maturity.
MINIMUM QUALIFICATIONS:
EDUCATION: Graduate of an accredited school of nursing and current licensure by Ohio State Board of Nursing
REQUIRED LENGTH AND TYPE OF EXPERIENCE:Two years recent Hospice, medical oncology or med/surg experience. Previous case management experience preferred.
REQUIRED LICENSURE, CERTIFICATION OR REGISTRY: C urrent license by the Ohio State Board of Nursing. CPR certification required. Must drive and have a valid Ohio Driver's license. CRNH preferred.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Conducts a comprehensive physical and psychosocial assessment of the patient according to established standard, including the assessment of Radiation specific side effects, pt safety and educational needs.
Participates in the establishment of a collaborative plan of care based on the assessment data, with all other involved disciplines and communicates this plan of care as appropriate.
Implements and evaluates the plan of care based on identified patient goals and intended outcomes. Modifies the plan of care, based on reassessment of the patient's status.
Completes all documentation according to established standard and utilizes the established documentation tools appropriately.
Assesses the readiness of the patient and significant other to learn and implements the individualized education plan according to the need.
Actively participates in the Department of Nursing or unit-based initiatives, including such activities as provision of in-service opportunities or volunteering to serve as project point for committees and task forces.
