Social Worker - Older Persons Phys Dis

Eden Brown is currently recruiting for a Social Worker - Older Persons Phys Dis to assess the needs of people in the area who require social care support and advice, you will need to provide services that meet the outcomes identified in the assessment and review services to ensure they continue to meet the identified outcomes, you will safeguard vulnerable adults All work to be carried out with users of our service, their families and carers within the framework of the seven social care outcomes, Improved health and emotional wellbeing Improved quality of life Making a positive contribution Exercising choice and control Maintaining personal dignity Economic wellbeing and freedom and freedom from discrimination.

Must have experience with Older Person with Mental Health( dementia/alzeihemers) and Physical disabilities. The successful candidate must have experience dealing with serious concerns and processes. The role will involve carrying out reviews for people in 24 hour care as well as home care reviews.

As part of the Agency Worker's Regulation from day one in their assignment you will be entitled to access collective facilities and internal vacancies.

Depending on the client & location as an agency worker where it is available you will also have access to the following facilities:
