SOCIAL WORKER needed for a CONTRACT opportunity with Yoh's client located in San Francisco, CA.

TOP SKILLS SHOULD YOU POSSESS:

- Clinical Social Work
- Case Management

WHAT YOU'LL BE DOING:

- Assist the care management team, providers and physicians in assessing and meeting the needs of patients
- Take a lead role in interviewing; assessing and diagnosing individual with complex behavioral health needs
- Develop integrated/individualized behavioral health treatment goals as part of a comprehensive care plan
- Provide guidance to non-licensed social work staff
- May conduct group or individual therapy depending on program needs indentified by Medical Services leadership
- Provide appropriate and timely documentation regarding complex biomedical and psychosocial issues
- Manage behavioral health referrals and provide navigation to patients and physicians to access appropriate services

WHAT YOU NEED TO BRING TO THE TABLE:

- Master's Degree in Social Work
- LCSW
- Five (5) plus years of experience in social services support in a health care setting
- Knowledge of Behavioral Health Issues
- Thorough knowledge of managed care, regulatory/accreditation and government entitlement programs
- Excellent verbal and written communication skills
