Social Worker | Disabled Children | Dorset

An exciting opportunity has arisen in the South West of England for a Qualified Social Worker in the Children with disabilities team.
The Qualified Social Worker will have strong experience of Social Work and have a proven track record of working within a Children's and Families team
They will be required to use professional knowledge to assess, monitor and review social care needs of individuals, relatives and carers to arrange the provision of appropriate care services

Duties will include:

*Interview clients individually, in families, or in groups, assessing their situations, capabilities, and problems, to determine what services are required to meet their needs.
*Devise individual care plans based on needs.
*Ability to liaise with other agencies to protect vulnerable people.
*Record all work and ensure that case notes that are recorded are clear and concise.

Experience/ Qualifications:

*Valid Social Work qualification.
*Minimum of 2 years post Qualified Social Work experience within a team.
*The Qualified Social Worker must also be HCPC registered and have an up to date CRB check.
*The candidate must be a car driver.
