Duties:
* Conducting community assessments
* Assessing for minor adaptations to patients homes
* Assessing for equipment aids and in depth DFGs
* Must be car driver due to travel at work.
Requirements:
You must have a relevant qualification in Occupational Therapy, be professionally registered with the HCPC and be eligible to work in the UK. You must have a minimum of 2 years recent post qualified experience working within a similar role. You must have experience of working within a community, intermediate care or social service environment. You must have a full clean driving licence.
