REGISTERED NURSE TELEPHONIC CASE MANAGEMENTneeded for a FULL TIME/CONTRACT opportunity with Yoh's client located in Downey, CA.

TOP SKILLS SHOULD YOU POSSESS:

- Registered Nurse in State of CA
- Two - five (2- 5) years of Case Management and Discharge Planning Experience in an Acute Care Setting
- InterQual Guidelines

WHAT YOU'LL BE DOING:

- Responsible for Utilization Management, Quality Referrals, and Discharge planning in an Acute Care and Managed Care Setting
- Demonstrate the knowledge and skills necessary to provide appropriate case management
- Assess discharge-planning needs of a diverse cultural and ethnic population
- Assigned patients ranging in age from neonates to geriatrics

WHAT YOU NEED TO BRING TO THE TABLE:

- Registered Nurse Licensure in State of CA (Active and in Good Standing)
- Bachelors Degree in Nursing preferred from Accredited Nursing School
- Certification in Case Management Preferred
- Two - five (2- 5) years Hospital/Inpatient and Managed Care Case Management and Utilization Management Experience
- Discharge Planning, InterQual and Milliman Experience
- Two - five (2- 5) years of direct patient care experience in ICU, Tele or ED required
- Intermediate level use of MS Word, Excel and Outlook
- Strong communication and interpersonal skills
