Registered Nurse (RN) Case Manager Job

Yoh has a contract opportunity for an RN CASE MANAGER to join our client in Fayetteville, NC.

JOB RESPONSIBILITIES:

- Provides clinical, casework and group work services
- Assesses the coordination and access to needed medical, psychiatric, social and educational services to meet the health care needs of the patient
- Evaluates cases and prepares and implements treatment and case management plans
- Counsels clients while assisting them to resolve problems and barriers to care
- Will maintain collaborative working relationships with other community providers
- Acts as a liaison between clients and providers
- Prepares and maintains written case records, reports and forms
- Performs case follow-up as needed

JOB QUALIFICATIONS:

- Associate Degree or equivalent years of experience
- Three (3) plus years of experience in case management
- Registered Nursing license in North Carolina
- Graduate from an accredited nursing program
- Worker's compensation case management experience
- Proficient use of Microsoft Word, Excel, Outlook, Electronic Medical Records

PREFERRED JOB SKILLS/KNOWLEDGE/EDUCATION:

- Bachelor's degree in Nursing (BSN)
- Clinical experience in occupational health preferred
