Registered Nurse-Health Coach Job

HEALTH COACH, REGISTERED NURSE (RN) needed for a 3 month maternity leave with Yoh's client located in New York, New York. This position will start MARCH 30TH!

- Health Coach, Health Fitness Specialist or certified Diabetes Educator

WHAT YOU'LL BE DOING:

- Provide acute, episodic, preventive and chronic care at the onsite health clinic
- Performs laboratory testing and administers injections and medications as ordered by the patient provider
- Provides face-to-face and telephonic lifestyle coaching, disease management and health education to patrons of the onsite health clinic to assist participants in making lasting behavior changes to achieve and maintain optimal health and wellbeing.
- Provides intake and scheduling duties

WHAT YOU NEED TO BRING TO THE TABLE:

- Degree in Nursing and current RN State licensure required
- Minimum of three (3) years of clinical experience as an RN in primary care, internal medicine, family practice, urgent care or office nursing settings required
- Well coaches Health Coach, Health Fitness Specialist or certified Diabetes Educator is highly desirable
- Excellent interpersonal, oral and written communication skills: mastery of public speaking
- Experience working with individuals to facilitate effective behavior change in the areas of physical, nutrition, stress management, and chronic condition management.

BONUS POINTS! OTHERWISE KNOWN AS PREFERRED QUALIFICATIONS:

- Position will require some travel between offices if needed
- RN- Licensed with State of NY
- Hours are Monday through Friday 8 am-5pm
