Registered Nurse - Health Coach Job

REGISTERED NURSE - HEALTH COACH needed for a contract opportunity with Yoh's client located in San Francisco, CA.

What You'll Be Doing:
- Responsible for providing care coordination and care management services
- Activities include: assessment, identification, planning, implementation of an individualized plan of care, and evaluation of outcomes
- Provide health coaching activities across the continuum of care in order to facilitate and promote high quality, cost effective outcomes, focused on the whole patient orientation and self management decision support and aims to minimize any fragmentation of health care delivery
- Acts as the liaison between the member, the member's support network, treating physician(s), and ancillary providers utilizing a multi-disciplinary approach to assess, identify, plan, health goal setting and measure optimal clinical outcomes
- Collaborate with internal and external stakeholders in coordinating a plan of care to achieve optimal quality healthcare outcomes

What You Need to Bring to the Table:
- Current California RN license
- Three (3) years of clinical setting experience as an RN health coach required (or equivalent experience)
- Managed care experience preferred, preferably in an HMO or IPA setting
- Knowledge of Manage Care concepts, health promotion strategies and education
- Excellent verbal and written communication skills
