Registered Nurse Case Manager Job

NURSE CASE MANAGER needed for a CONTRACT opportunity with Yoh's client located in Salinas, CA.

TOP SKILLS SHOULD YOU POSSESS:

- Case management
- Utilization management

WHAT YOU'LL BE DOING:

- Manage care plans for members referred into the care management program
- Work with and educate members, providers, external agencies and internal departments on care management program and provide assistance as needed
- Participate in Quality Improvement studies, to continually evaluate program effectiveness and ability in promoting quality driven, cost effective achievable goals and outcomes

WHAT YOU NEED TO BRING TO THE TABLE:

- Current California RN license
- A minimum of three (3) years of experience in a patient care setting, including at least one (1) year of case management experience, preferably in a managed care environment
- Knowledge of utilization management principles and activities
- Knowledge of case management principles and activities
- Knowledge of Medi-Cal and related policy and Title 22 regulations
- Knowledge and understanding of concepts pertaining to prepaid health care

BONUS POINTS! OTHERWISE KNOWN AS PREFERRED QUALIFICATIONS:

- Bilingual English/Spanish or Hmong is preferred
