Registered Nurse Case Manager Job

Yoh has a contract opportunity for an RN (REGISTERED NURSE) CASE MANAGER to join our client in San Ramon, CA.

JOB RESPONSIBILITIES:

- Will provide telephonic medical case management for worker's compensation injury programs.
- Accurately and efficiently identify properly requested files for triage.
- Process files for determination of telephonic or field case management.
- Complete referrals in database from injury report.
- Review and evaluate all medical correspondence and provider reports.
- Review treatment plan history and direction of file with client as needed.
- Will evaluate clinical status of claimant and researches alternate options for treatment.
- Arrange for care/services as needed for claimant.
- Identify any cases that are not utilizing MPN or PPO providers.
- Other duties as assigned.

JOB QUALIFICATIONS REQUIRED:

- California RN license.
- Graduate of an accredited school of nursing.
- Three to five (3 - 5) years of recent experience in Case Management.
- Worker's Compensation case management experience preferred.
- Proficient use of Microsoft Word, Excel, Outlook.
