RESPONSIBILITIES;
- Lead and manage the Reablement Services provision across the Authority, ensuring planned outcomes/specified performance targets are met.
- In conjunction with the Head of Social Care, to develop performance targets/measurable outcomes within business plans which seek continuous improvement in the overall efficiency and effectiveness of operations and deliver on these.
- To ensure that appropriate information is maintained to monitor and manage delivery of these targets.
- Line management of Reablement Services and any other services as specified by the Head of Social Care
- Manage all budgets associated with Reablement Services, ensuring that resources are used effectively and that balanced budgets are delivered at year end.
- Lead and manage the further establishment and development of Reablement Services and the 'prevention, reablement and early support' ethos across the Social Care and Support Service.
- To ensure that monitoring and action plans are in place to maintain and update required registrations under the Care Quality Commission (CQC) frameworks, or any body which replaces CQC.
- Work in collaboration with internal and external partners to ensure integrated services are developed and successfully delivered, in line with customer expectations, legislative requirements and the Council's philosophy and vision.
This role is for an initial 6 month period, starting at the end of September.
