Reablement & Reviews Social Worker

Eden Brown Recruitment require your help filling the following HCPC registered social worker position.

This position is based within the Information & Assessment team.

Please see below for the required experience which is essential:

- 2-3 years post qualifying experience
- experience of Information & Assessment team (ideally within the last 12 months)
- experience of reablement & reviews
