Qualified Social Workers - Response and Reablement

Our client has job vacancies for Qualified Social Workers to work within a Response and Reablement Team based in South London. These vacancies have an initial 3 month contract and the pay rates for these roles are £25-28 per hour depending on experience (limited company worker).

Requirements:
Have you got a social work degree or equivalent?
Are you HCPC registered?
Do you have 2 years experience within in the UK?
Do you have experience working within an integrated health and social care environment?
Do you have experience with hospital discharge within community social work?

If you fit these requirements then you could be the ideal candidate.

Duties:
- You will be facilitating discharges from hospital.
- You will be managing a caseload.
- You will be carrying out assessments.
