Our client has a job vacancy for a Qualified Social Worker to work within a Transitions Team based in South London. This vacancy has an initial 3 month contract and the pay rate for this role is £24-28 per hour depending on experience (limited company worker).

Requirements:
Have you got a social work degree or equivalent?
Are you HCPC registered?
Do you have 2 years experience within Adult Services in the UK?
Are you familiar with the Mental Capacity Act?
Do you have an understanding of transition from Children's to Adult Services?

If you fit these requirements then you could be the ideal candidate.

Duties:
- You will be taking on an active caseload.
- You will be setting up care packages.
- You will be undertaking safeguarding adult investigations.
- You will be assessing needs.
