To undertake the assessment, support planning and care co-ordination of vulnerable adults with complex needs within a team environment.

The following tasks are indicative of those that may be expected from the post holder and are not a complete list of those that a post holder may undertake.
To undertake the assessment, support planning and care co-ordination of vulnerable adults with complex needs.
To provided advice and support to service users, their families and as appropriate the public on a day to day basis.
To promote close and constructive relationships with service users, carers and partner agencies.
To identify and highlight resource issues to the Team Leader and Senior Practitioner.
To promote standards which achieve equality of opportunity
To ensure personal adherence to all relevant Policies & Procedures.
