Carrying out assessment of needs following Section 2 notification for discharge
*Setting up care packages or preparing panel documents for funding applications to 24 hour care placements under the Delayed Discharge Act 2003 and the Fair access to care services policy
*Liaising and negotiating with multidisciplinary team and placement, brokerage teams
*Designing care packages to meet clients needs in the community or care home placements
*Working in partnership with multidisciplinary teams in local and out of borough hospitals, service users and carers
Achieve target within section 5 to prevent any reimbursement fee.
