QSW | Transitions | Buckinghamshire

We have a rare opportunity for a Qualified Social Worker to join the Transitions Team in Buckinghamshire, paying up to £30/hr.

The Transitions Team works with young adults with disabilities aged between 14-25 and their carers. You will be required to support young people to achieve their full potential to be as independent as possible. You will work with young people and their carers to provide person centred care assessments.

This is a rewarding position where you can be involved in transforming the lives of service users and their families.

Ideally you will have experience of working in children's and adult services, including learning disabilities. Experience and knowledge of working with children with disabilities would be an advantage.

You should hold a recognised Social Work Qualification, be HCPC registered and have access to your own transport.
