QSW - Leighton buzzard - Older People Physical Disabilities Team - £28 Per Hour Ltd

We are looking for an experienced social worker with community based Older People & Physical Disability background.
The successful candidate will need previous knowledge of SWIFT software would be an asset.
The role requires you to work with service users to assess their needs, identify options for meeting these needs, negotiate and review appropriate care within the department's priorities.
To work with service users and/or liaise with service providers from within the department or other agencies, as necessary.

The Role
To facilitate and support service users
To undertake supported self-assessments and reviews to identify needs and outcomes.
Providing assessments of need taking into account current support systems and to evaluate potential risks, for example Mental Capacity Act, continuing health care and safeguarding.

Job Purpose
To promote a model of self-directed support to service users and carers to determine their own solutions. To work generically as a member of a multi-disciplinary team within the self-directed support model using the principles of personalisation.

You will be responsible for
Undertaking comprehensive multi-disciplinary assessments under the NHS and Community Care Act 1990, of people living in the community of Bedfordshire and using a person centred approach to identify eligible needs.

Agreeing a package of care alongside the service users and /or family members or carers which helps the person remain in their own home or if this is not possible, to move to a suitable 24 hour care environment.
Carrying a caseload of service users (older people and people with a physical disability) and through the Care Management process provide advice, support and professional expertise with the aim to promote independence and maximise potential. Part of this caseload will involve long term professional support for a number of very vulnerable people whose condition is complex, unstable or deteriorating and/or where there are complex support network dynamics.

Enhanced DBS
HCPC registered
Car driver/ owner
Post Qualified experience working within this setting
