Programme Lead for Long Term Conditions

My NHS client is looking for a Programme Lead for their Long Term Conditions project. The programme lead will assist with initiating the significant shift in resources away from the hospital sector to community based services, identifying and case managing patients, based on a risk stratification.
The Programme Lead will need to have good knowledge of the following work streams, including, self care, telehealth, copd, dementia, diabetes, heart failure, falls.
This six month period will include the development of an overarching business case to support the roll out of the programme.
The Programme Lead will need to have a confident approach and be able to engage with people at all levels and have good strategic thinking ability. It is essential that the individual has good programme management experience. This programme has buy in from key stakeholders, and the investment is already underpinned by significant transition funding. What is now required is the focus to push things forward.

Essential skills:

- Sound project/programme management experience
- Good understanding of Long Term Conditions
- Good written skills
- Ability to interact with people at all levels
- Ability to lead

This role is a full time 6 month post, paying £450 - £500 per day.
