Sand Resources is looking for an NHS Whole Systems Care Pathway Manager.

This role is to effectively develop the administration and discharge processes between acute and community care. You will be responsible for reducing the need for unplanned administrations and timely discharges by enabling safe processes and liaising with intermediary and community care services.

The successful candidate will be acting as the bridge between acute and community care services and will be required to develop the risk management processes associated with transfer/discharge.

You will have a knowledge of the key issues between primary and secondary care as well as a good understanding of care pathways. You will also have a sound knowledge of intermediate care and clinical governance procedures.

You will have recently worked in a role that focused on discharge processes and preventing unnecessary admissions as well as having experience of working with community care services.

Key Responsibilities:
- To support the MDT discharge decision making process
- To make critical judgements involving highly complex factors
- To prevent avoidable admission to acute care
- To prevent delayed discharge by liaising with community practitioners and Rapid Response Teams
- To enable the process of safe discharge from hospital
- To promote a seamless and multi-disciplined approach to health and social care
- To develop intermediary care services
- To act as a bridge between acute and community care
- To develop risk management procedures
