POSITION INFORMATION

-POSITION SUMMARY:

The Occupational Therapist is an important member of the home health care rehabilitation team, with expertise in maximizing the patient's safety and independence in functional activities of life, including activities of daily living (ADLs), such as dressing and bathing; instrumental activities of daily living (IADLs) such as meal preparation and laundry. The role plans, provides and directs patient care, in accordance with home health patient care standards, regulations, and physician orders. The OT acts as a liaison between physician, patients and family, and collaborates with the home health care team. The OT must be able to demonstrate the knowledge and skills necessary to provide care appropriate to age groups according to chronological age, developmental age, and/or psycho-social maturity.

MINIMUM QUALIFICATIONS

-EDUCATION:
-Graduate of an accredited Occupational Therapy program with a B.S. or certificate in Occupational Therapy.
-REQUIRED LENGTH AND TYPE OF EXPERIENCE:
-

Prefer one year of experience in acute, rehabilitation, or home health setting.

-REQUIRED LICENSURE, CERTIFICATION OR REGISTRY:
-

Current Ohio licensure and CPR certified.

PRINCIPLE DUTIES AND RESPONSIBILITIES

-Below are the duties and responsibilities for this position 
- Optimize patient's functional performance rather than dependence on others.
- Interventions utilized by the OT includes: upper extremity hand therapies, sensorimotor training, energy conservation, and modification of the home environment.
- Train patient's in the use of adaptive devices or compensatory techniques.
- Prepare clinical documentation, attend case conferences, and participate in care planning.
- Provides ongoing input into the plan of care, particularly cases with a Home Health Aide assigned.
- Communicates to home care team and managing physician on a regular basis about the patient's progress.
- Supervise and direct Occupational Therapist Assistants.
