POSITION SUMMARY:
Is an important member of the home care rehabilitation team, with expertise in maximizing the patient's safety and independence in functional activities of life, including activities of daily living (ADLs), such as dressing and bathing; and instrumental activities of daily living (IADLs) such as meal preparation and laundry. The role plans, provides, and directs patient care, in accordance with home health patient care standards, regulations, and physicians orders. The OT acts as a liaison between physician, patients and family, and collaborates with the home health care team. The OT must be able to demonstrate the knowledge and skills necessary to provide care appropriate to age groups according to chronological age, developmental age, and/or psycho-social maturity.

MINIMUM QUALIFICATIONS:

EDUCATION, KNOWLEDGE, SKILLS AND ABILITIES: Graduate of an accredited Occupational Therapy program with a B.S. or certificate in Occupational Therapy.

REQUIRED LENGTH AND TYPE OF EXPERIENCE: Prefer one year of experience in acute, rehabilitation, or home health setting.

REQUIRED LICENSURE OR CERTIFICATION: Current Ohio licensure and CPR certified.
