Occupational Therapist Community Health, Regional NSW Level 2-3

My client is seeking for a level 2- 3 OT who is willing to relocate in Regional NSW and grow with their community health providing support and service to the local health district and the outer community.
A) Relevant tertiary qualifications in the field of Occupational Therapy, current registration as an Occupational Therapist with AHPRA, eligibility for membership of the Australian Association of Occupational Therapy, and a current unrestricted drivers licence (recognised in Australia).
B) Demonstrated experience in the delivery of successful community health assessment, treatment and care planning services, especially in a rural setting.

Key Accountabilities:
1. Assessment and intervention (including rehabilitation therapy) to clients who are referred to the Specialist Aged Care & Rehabilitation Team.
2. Prescribe and train clients and carers in the use of aids and equipment for daily living.
3. Prescribe home modifications to assist the client/carers to maintain a client at home.
4. Establish Occupational Therapy treatment goals and recommendations in consultation with
clients and their carers.
5. Provide case coordination for clients including referral to other services for clients, as
necessary.
6. Liaise with case managers, other allied health staff, GPs and community services to provide a
coordinated care approach and reinforce recommendations, where appropriate.
7. Develop relationships with other teams such as ACAT, community nursing, Connecting Care, Community Options and other HACC funded services.
8. Ensure appropriate clinical records are maintained.
9. Attend relevant meetings eg. Specialist AC&R Team meetings, case conferences, OT meetings, necessary for efficient performance of duties.
10. Meet NSW Health, SWS LHD, COAG and local data requirements.
11. Coordinate placement of and supervise students on clinical placements, where necessary.
12. Promote evidence based practice in the Occupational Therapy service.
13. Identify areas of need for quality improvement projects and assist with
implementation of quality projects.
14. Develop educational materials and participate in the education of the community, clients, staff and carers.
15. Participate in health promotion activities, as required.
16. Contribute to personal professional development by attending relevant training and identifying
and accessing self-education to ensure current knowledge of developments in relevant areas .
17. Act as a resource person for the profession in the community.
