Occupational Therapist Community

We are looking for an experienced Band 6 occupational therapist to work in the community. You will be based in the REDS team which inreaches into the local acute hospitals and identifies, assesses and organises the discharge home of patients who are medically fit for discharge but awaiting referral to therapy, social services or for home visits and completes this getting the patient home earlier than would normally happen setting goals, organising care packages and ensuring the home environment is safe for discharge. The job can be fast-paced, often busy but very rewarding.

Hours: 37.5hrs (includes some weekends)
