STROKE COORDINATOR REGISTERED NURSE (RN) OR NP NURSE PRACTITIONER needed for a CONTRACT opportunity without client located in Northridge, CA.

The Big Picture:
- The Program Manager/Stroke Coordinator is responsible for coordinating and implementing services and activities related to the Stroke Center. He/she functions as a liaison between the physicians, nursing staff, allied healthcare professionals, patients and their families. He/she coordinates and facilitates the care for the stroke patient throughout the continuum.
- The Program Manager/Stroke Coordinator works closely with physicians and nursing to ensure clinical care paths are followed, as appropriate. The Program Manager/Stroke Coordinator participates in program development as it relates to the stroke center. This includes, but is not limited to staff education, community education, and marketing. Responsible in coordination with the Director, for planning, developing and implementing stroke center Our client is seeking an RN with strong clinical skills, good communication skills. You must have prior Disability Case Management experience to be considered.

What You'll Be Doing:
- Review patient's medical history and performs physical assessment within scope of practice. Performs patient physical assessment for stroke patients, in coordination with attending physician, according to privileges.
- Writes, reviews and implements physician orders within scope of practice.
- Takes call for response to code stroke (RRT or ED admit) while on-site.
- Provides for patient/family/Care Partner education in coordination with nursing staff.
- Identifies patient/family/Care Partner psychosocial needs (ie cultural, spiritual) and addresses as applicable.
- Coordinates overall care for stroke patient in coordination with physician and nursing, driving length of stay.
- Acts as a liaison between physicians, nursing and ancillary staff and patients and their families/Care Partners.
- Completes hospital and department documentation, legibly, accurately and within designated time frame.
- Provides clinical resources and consultation to staff regarding stroke patients with clinical complex needs.
- Works with nurse educators and director to implement formal and informal nurse education and training regarding stroke care.

What You Need to Bring to the Table:
- Must have minimum five (5) years nursing experience, including care of the stroke patient.
- Two (2) years of experience in a managerial or program development position required. Specialized Training: Must have working knowledge of performance improvement, licensure and accreditation requirements.
- Demonstrates good capacity for teaching, organizing, planning and development. Strong interpersonal communication and customer relation skills required. Licensure: Must have current CA RN license, current nurse practitioner certification, BLS, ACLS. Software: Knowledge of Microsoft Office, Outlook, and Internet.
