Nurse Practitioner or Physician Assistant Job

NURSE PRACTITIONER OR PHYSICIAN ASSISTANT needed for a contract opportunity with Yoh's client located in Allendale, SC.

THE BIG PICTURE:

- Home visit 30 hours/week

TOP SKILLS YOU SHOULD POSSESS:

- One to three (1-3) years of experience in Home Visits
- Medicare Experience

WHAT YOU'LL BE DOING:

- 30 hours a week home visits for Medicare Patients- Ability to obtain reliable transportation in order to complete home visit assessments
- Knowledgeable in regards to standard of care of common chronic health conditions as well as health promotion and prevention guidelines

WHAT YOU NEED TO BRING TO THE TABLE:

- Nurse Practitioner or Physician Assistant in State of South Carolina (Active and in Good Standing)
- Two to five (2-5) years of Experience Home care or home visit experience
- Family, Geriatrics, or Adult certification required
- Must have Strong communication skills with the geriatric or Medicare population
- Must have reliable Transportation and be willing to go to different home visits
- Prescriptive Authority in State of SC
- Professional image and behavior are required
- Ability to navigate on the Internet and work with personal computer
- Knowledgeable in regards to standard of care of common chronic health conditions as well as health promotion and prevention guidelines
